The series will comprehensively cover the process of case management, introducing necessary skills and supportive tools to utilize in providing best practices related to case management. The phases will feature an in-depth dive with scenarios to practice refining case management skills. Each of the trainings will cover topic-specific documentation tips.

Monitoring and Evaluation: Identify different tools/ways to monitor progress during this case management phase, along with strategies to adjust the plan when needed. We will review the transition process and identify signs of readiness for the Transition phase.
